The Chain of Command: The HPG Axis

Mnemonic for the flow: Great Fun, Let's StartGnRH → FSH/LH → Steroids.

Feedback: the thermostat logic (HOW it stays controlled)

Almost all of reproduction runs on negative feedback to keep hormone levels stable. There is one famous exception: the LH surge at ovulation, which is positive feedback.

Male hormonal control (steady state)

Female hormonal control (the cycle)

The ~28-day menstrual cycle has two halves separated by ovulation.

Active recall

Recall Feynman: explain to a 12-year-old

Your brain is the boss. It whispers a small order (GnRH) to a middle manager (pituitary), who shouts louder orders (FSH & LH) to the factory (ovaries/testes). The factory makes the real products (eggs/sperm + hormones). The products report back to the boss: "we have enough, slow down" — that's the brake (negative feedback). Once a month in girls, the factory shouts "the egg is READY!" so loudly that the boss says "GO, GO, GO!" instead of slowing down — that one loud burst (LH surge) pops the egg out. That single "shout louder" moment is positive feedback.

Connections

What are the three levels of the HPG axis?
Hypothalamus → Anterior Pituitary → Gonads
Which hormone does the hypothalamus release to start the axis?
GnRH (Gonadotropin-Releasing Hormone)
What two gonadotropins does the anterior pituitary release?
FSH and LH
In males, LH acts on which cells to do what?
Leydig cells → produce testosterone
In males, FSH acts on which cells to do what?
Sertoli cells → support spermatogenesis
Which hormone selectively inhibits FSH in males?
Inhibin (from Sertoli cells)
What does FSH do in the follicular phase?
Stimulates a follicle to grow and secrete oestrogen
What causes ovulation?
The LH surge, triggered by sustained high oestrogen (positive feedback)
What is the feedback type behind the LH surge?
Positive feedback (the one reproductive exception)
What does the corpus luteum secrete and why?
Progesterone (+ oestrogen) to maintain the uterine lining
What happens to hormones if no pregnancy occurs?
Corpus luteum degenerates → progesterone & oestrogen crash → menstruation → FSH rises for next cycle
How does the combined pill prevent pregnancy?
Constant oestrogen+progesterone → negative feedback lowers FSH/LH → no follicle matures, no LH surge, no ovulation
What rescues the corpus luteum in early pregnancy?
hCG from the embryo, later the placenta
Why is male control "steady" and female "cyclic"?
Males use flat negative feedback (continuous sperm); females add a monthly positive-feedback surge to release one egg

Dekho, reproduction ka control ek chain of command ki tarah chalta hai jise HPG axis kehte hain. Sabse upar hai dimaag ka hypothalamus, jo GnRH nikaalta hai. Ye GnRH neeche pituitary ko bolta hai ki FSH aur LH release karo. Aur ye FSH/LH gonads (testes/ovaries) ko bolte hain ki gametes banao aur sex hormones (testosterone ya oestrogen/progesterone) chodo. Teen level isliye hai taaki chhota sa signal amplify ho jaye aur har step pe control (feedback) mil sake.

Sabse important concept hai feedback. Zyada tar time negative feedback chalti hai — matlab final hormone upar wapas jaake bolta hai "bas, ab band karo", thermostat ki tarah. Lekin ek special exception hai: ovulation ke time positive feedback. Jab follicle mature hoke bahut zyada oestrogen banata hai aur ye level lambe time tak high rehta hai, toh pituitary ka reaction ulta ho jaata hai — high oestrogen ab LH ko badhata hai, jisse ek bada LH surge aata hai aur egg release ho jaata hai. Isko yaad rakho: "High O, LH GO."

Females mein cycle do halves mein: pehle follicular phase (FSH follicle badhata, oestrogen banta), phir ovulation (LH surge), phir luteal phase jisme bacha hua follicle corpus luteum ban ke progesterone banata hai jo uterus ki lining maintain karta hai. Agar pregnancy nahi hui, corpus luteum mar jaata hai, progesterone gir jaata hai, aur periods aa jaate hain — aur brake hatne se FSH phir se badhta hai, next cycle shuru. Males mein koi surge nahi, bas flat negative feedback, kyunki sperm continuously banta rehta hai. Yehi logic contraceptive pill mein use hota hai: constant oestrogen+progesterone dete hain, negative feedback se FSH/LH dab jaate hain, follicle mature nahi hota, ovulation nahi hoti — pregnancy rukti hai.
